Registry-based trials (RBTs) leverage existing clinical registries to offer faster, cost-effective alternatives; enhancing Australia's research capacity. However, RBTs face methodological and operational challenges. This presentation will provide a detailed look at a project focused on creating a best practice guide to support the planning, execution, and documentation of RBTs. The development of the guide was informed by (i) Scoping review, (ii) National multi-stakeholder expert meeting, and (iii) Semi-structured key informant interviews. Findings from this project also highlight key enablers and challenges to the design and conduct of RBTs in Australia. The resultant guide offers key considerations for trialists and registry operators in designing, conducting, and reporting RBTs.
